新型冠状病毒疫苗接种发生皮肤不良反应的临床分析

摘要： 本研究分析了2021年10月至2022年10月我院COVID-19疫苗接种后引起的106例皮肤不良反应。其中，第一剂，第二剂和第三剂COVID-19疫苗接种后皮肤不良反应发生率分别为58.49%、25.47%和16.04%；且既往皮肤病患者接种后的不良反应占所有皮肤不良反应的19.81%。荨麻疹样皮疹是接种疫苗后最常见的皮肤不良反应，其后为湿疹样皮疹和带状疱疹发生率为42.45%，30.19%和11.32%。COVID-19疫苗接种后其它罕见的皮肤不良反应包括玫瑰糠疹，多形红斑，水痘，银屑病，红皮病，麻疹样皮疹和白癜风。在三剂COVID-19疫苗的接种中18例患者出现2次皮肤不良反应，3例患者出现3次皮肤不良反应。针对这些皮肤不良反应，治疗措施大多是对症处理，如口服抗组胺药氯雷他定片等，外用弱至中效糖皮质激素药膏，大约2~3周可治愈。

Abstract: The data of 106 cases of skin adverse reactions after vaccination of COVID-19 vaccine in our hospital from October 2021 to October 2022 were analyzed. The incidence of skin adverse reactions after the first, second and third doses of COVID-19 vaccine were 58.49%, 25.47% and 16.04% respectively. In addition, the adverse reactions of patients with skin diseases after vaccination accounted for 19.81% of all skin adverse reactions. Urticaria-like rash was the most common skin adverse reaction after vaccination, followed by eczematoid rash and herpes zoster with an incidence of 42.45%, 30.19% and 11.32%. Other rare skin adverse reactions after COVID-19 vaccination included pityriasis rosea, erythema multiforme, chickenpox, psoriasis, erythroderma, measles like rash and vitiligo. Among the patients with three doses of COVID-19 vaccine, 18 patients had two skin adverse reactions, and 3 patients had three skin adverse reactions. For these skin adverse reactions, most of the treatment measures were symptomatic treatment, such as oral antihistamine loratadine tablets, and external use of weak to medium effect corticosteroid ointment, the patients could be cured in about 2-3 weeks.
