长脉宽1064nmNd:YAG激光治疗婴儿血管瘤疗效评价

Assessment of long pulse width 1064 nm Nd:YAG laser in the treatment of infantile hemangioma

摘要: 目的:评价长脉宽1064nmNd:YAG激光治疗婴儿血管瘤的临床疗效及不良反应。方法:回顾性分析61例经长脉宽1064nmNd:YAG激光治疗的婴儿血管瘤,治疗参数:3mm光斑,脉宽30-40ms,能量密度180-220J/cm2,DCD30/20/30ms,间隔时间为4周,治疗次数1-7次不等,所有患儿均为浅表血管瘤的隆起性皮损。 结果:57例患儿皮损改善76%~100%,4例改善51%~75%。不良反应包括: 水疱20例 (32.79%),1天~10天修复;浅表瘢痕9例(14.75%);色素减退5例(8.20%);色素沉着4例(6.56%);毛发稀少或缺失3例(4.92%)。随访3个月时色素改变及瘢痕均有不同程度的改善,但毛发改变无明显改善。结论:长脉宽1064nmNd:YAG激光治疗婴儿血管瘤有效,但不良反应率较高。

Abstract: Objective: To assess the clinical efficacy and adverse reaction of long pulse width 1064nmNd:YAG laser in the treatment of infantile hemangioma. Methods: The data of sixty-one cases with infantile hemangioma treated with long pulse width 1064nmNd:YAG laser with 3mm light spot, pulse width 30-40ms, energy density 180-220J/cm2, DCD30/20/30ms, once 4 weeks were analyzed retrospectively. The hemangioma in all patients was protuberant and superficial. Results:The improvement of 76%~100% was seen in 57 cases and of 51%~75% in 4 cases. The adverse reactions included transient blister, which was subsided during one to 10 days in 20 cases (32.79%), superficial scar formation in 9 (14.75%), depigmentation in 5 (8.20%), pigmentation in 4 (6.56%) and sparse hair or absence in 3 (4.92%). After 3 months of follow-up, pigmentation and scar were improved, but the sparse hairs were not significantly improved. Conclusion: Long pulse width 1064nmNd:YAG laser is an effective method in the treatment of infantile hemangioma, but the adverse reactions are more.
