扩张器在治疗儿童四肢先天性色素痣中的应用

Application of skin expander in the treatment of congenital nevus located on children's extremities

摘要： 目的：评价扩张器用于修复儿童四肢先天性色素痣的临床效果。方法：I期手术：在色素痣的上下或左右正常皮肤下放置10～300 mL扩张器1～3个。II期手术：取出扩张器，根据色素痣面积及形状设计局部推进或旋转皮瓣修复皮肤缺损。观察并记录术后皮瓣成活情况。结果：应用扩张皮瓣修复四肢先天性色素痣患儿30例，共埋置扩张器34个，其中成功扩张27个，扩张囊外露3个，因扩张器感染无法继续扩张4个。在成功扩张的27个皮瓣中，26例皮瓣成活，修复后形态自然，1例皮瓣尖端局部坏死。结论：扩张器治疗儿童四肢先天性色素痣安全有效。

Abstract: Objective: To assess the application of dilator in repairing congenital nevus on extremities in children. Methods: In the first stage, 1 to 3 cylindrical expanders with 10 to 300 mL were placed at left or right sides or at upper or lower parts of the nevus. In the second stage, nevus were repaired with expanded transverse propulsive and lateral or rotary flaps, according to the areas and shapes of the scars. The survival rate of the flap was assessed. Results: Thirty cases of congenital nevus on the extremities were treated with expanded skin flaps and 34 skin expanders were implanted. Of 34 skin expanders, 27 were successfully expanded, 3 exposured and 4 were infected. Of 27 flap successfully expanded, 26 survived with satisfactory function, and 1 flap has local necrosis on the edge. Conclusion: The application of dilator in congenital nevus is safe and effective.
