皮肤Rosai-Dorfman病一例并文献回顾性分析

Cutaneous Rosai-Dorfman disease: a case report and a retrospective analysis of the literature

摘要: 目的:皮肤Rosai-Dorfman病(cutaneous Rosai-Dorfman disease,CRDD)是一种罕见的组织细胞增生性疾病,本文对1例患者资料进行报道,并对既往文献进行回顾性临床病理分析。方法:通过PubMed查询公开发表的英文文献,对该病的人口统计学信息、临床表现、病理特点及治疗等情况进行分析总结。结果:共检索到符合标准的183篇文献,其中包括371例患者资料。分析结果显示男女比例为1∶1.1,平均年龄为46岁,以东亚人(52.3%)最为常见。皮损主要表现为多发的结节、斑块、丘疹,常见于四肢(43.1%)、躯干(36.9%)及面部(27.2%)。该病诊断主要依靠组织病理及免疫组化检查。疾病通常呈慢性良性病程。结论:CRDD好发于东亚人群,具有一定的临床和组织病理学特点,皮肤科医生应该提高对该病的认识。

Abstract: Objective: Cutaneous Rosai-Dorfman disease (CRDD) is a rare histiocytosis. The purpose of this study was to characterize CRDD through a retrospective clinicopathological analysis of the literature and present a new case. Methods: A literature search was performed on PubMed and information analysis was performed, which included demographics, clinical characteristics, histology, immunohistochemistry and treatment. Results: There were 183 studies available for analysis that included 371 cases. The male: female ratio was 1∶1.1, and the average age was 46 years. The disease is more common in Eastern-Asians (52.3%). The most commonly affected skin regions were limbs(43.1%) trunk(36.9%) and face (27.2%) and most lesions were multiple nodules, plaques and papules. The diagnosis of the disease mainly depends on histopathology and immunohistochemistry. The disease shows an indolent and benign course. Conclusion: Cutaneous Rosai-Dorfman disease is more common in East Asian population and it has certain clinical and pathological features. Dermatologists should improve the understanding of the disease.
