三种梅毒螺旋体特异性抗体检测方法性能评价

摘要: 目的:比较电化学发光免疫分析(ECLIA)、化学发光免疫分析(CLIA)、酶联免疫吸附实验(ELISA)三种方法检测梅毒螺旋体特异性抗体的性能差异。方法:通过梅毒螺旋体抗体颗粒凝集试验(TPPA)实验筛选阳性样本和阴性样本各60例,对所有样本的梅毒螺旋体特异性抗体采用三种方法检测,通过受试者工作曲线(ROC)确定不同方法学对梅毒的诊断价值,并确定S/CO最佳截断值,比较不同方法学AUC面积。结果:当ECLIA法S/CO≥5或CLIA法S/CO≥7或ELISA法S/CO≥7时,TPPA法复查检测结果均为阳性。ECLIA法、CLIA法诊断梅毒的灵敏度、特异性、AUC及符合率均大于ELISA法,差异均有统计学意义(均P<0.05)。结论:当ECLIA、CLIA、ELISA法S/CO分别小于5、7、7时,需采用TPPA进行验证,以减少误诊。ECLIA法、CLIA法诊断价值高于ELISA法。

Abstract: Objective: To compare the performance differences of three methods (ECLIA,CLIA, ELISA) in the detection of Treponema pallidum specific antibody. Methods: 60 cases of positive samples and 60 cases of negative samples were screened by TPPA test, all samples were tested for Treponema pallidum specific antibody by ECLIA, CLIA and ELISA. By using receiver operating curve (ROC) to define the value of different methods in the diagnosis of syphilis, determine the best truncation value of S/CO, and compare AUC size of different methods. Results: When S/CO ≥5 of ECLIA, S/CO≥7 of CLIA or S/CO≥7 of ELISA, TPPA were ture positive. The AUC, sensitivity, specificity, and coincidence rate of ECLIA and CLIA were higher than those of ELISA, with significant differences (Ps<0.05). Conclusion: When the S/CO of ECLIA, CLIA and ELISA is less than 5, 7 and 7 respectively, TPPA should be used for verification to reduce misdiagnosis. The diagnostic value of ECLIA and CLIA is higher than ELISA.
