卡马西平致HLA-A*31:01基因携带者发生超敏反应综合征一例

Carbamazepine-induced DRESS syndrome with HLA-A*31:01 gene positive

摘要: 目的:明确一例可能由卡马西平诱发的药物超敏反应综合征的诊断,确定致敏药物。方法:收集患者临床资料,提取患者外周血单个核细胞,进行HLA等位基因分型和淋巴细胞转化实验(lymphocyte transformation test, LTT)。结果:该患者HLA等位基因分型提示患者携带HLA-A*31:01;LTT结果显示在卡马西平刺激后,淋巴细胞进行增殖。结论:结合患者临床表现、HLA分型和LTT结果,诊断为由卡马西平诱发的药物超敏反应综合征。

Abstract: Objective: To clarify the diagnosis and identify sensitizing drugs in a patients with eosinophilia and systemic symptoms (DRESS) syndromedrug hypersensitivity syndrome possibly induced by carbamazepine. Methods: Clinical data and the peripheral blood of the patient were collected and the peripheral blood mononuclear cells (PBMCs) were extracted. HLA testing and lymphocyte transformation test (LTT) were performed. Results: The HLA testing indicated that the patient carried the HLA-A*31:01 allele. The LTT results of this patient showed that lymphocytes proliferated after carbamazepine stimulation. Conclusion: Based on the clinical manifestations, HLA testing and LTT results, the patient was diagnosed as the DRESS syndrome induced by carbamazepine.
