砷角化病继发Merkel细胞癌合并鳞癌一例并文献复习

Arsenic keratosis with secondary Merkel cell carcinoma and squamous cell carcinoma: a case report and literature review

摘要: Merkel细胞癌(MCC)是一种罕见的高侵袭性皮肤神经内分泌癌,目前关于砷暴露相关MCC报道仅16例。本文病例为73岁男性患者,全身多处皮肤肿物,反复溃烂、渗液7年余。7年前有含硫化砷药膏外用史。皮肤科查体见掌跖部广泛角化过度的基础上出现多发深黄色角化性丘疹,全身皮肤散在色素沉着斑,脐下见一隆起肿物,边界清,表面见糜烂渗出;左膝、右大腿上段及下段皮肤见多处灰白色粗糙区,触之易出血,轻压痛。经组织病理检查确诊为砷角化病、腹部MCC及下肢多发性鳞状细胞癌。

Abstract: Merkel cell carcinoma (MCC) is a rare and highly aggressive cutaneous neuroendocrine carcinoma. To date only 16 cases of MCC associated with arsenic exposure have been reported in the literature. The patient in this case was a 73-year-old male who presented with multiple cutaneous tumors all over the body, accompanied by repeated ulceration and exudation for more than 7 years. Seven years prior, he had a history of topical application of arsenic sulfide-containing ointment. Dermatological examination revealed multiple deep yellow keratotic papules on the basis of extensive hyperkeratosis of the palms and soles, scattered pigmented macules on the entire skin, and an elevated tumor under the umbilicus with clear boundaries and eroded exudation on the surface. Multiple grayish-white rough areas were observed on the skin of the left knee, upper and lower segments of the right thigh, which were prone to bleeding on palpation with mild tenderness. The diagnosis of arsenic keratosis, abdominal MCC, and multiple squamous cell carcinoma of the lower extremities was confirmed by histopathological examination.
