摘要:

目的:分析慢性荨麻疹患者病情严重程度与血清免疫五项的相关性。方法:对160例慢性荨麻疹患者进行荨麻疹活动度评分(Urticaria Activity Score,UAS)和皮肤病生活质量评分(Dermatology Life Quality IndexDLQI),判断患者病情严重程度并分别检测其血清免疫五项指标。80例正常对照组的血清免疫五项指标作对照。对临床病情严重程度和检验结果的相关性进行了分析。 结果:慢性荨麻疹患者血清IgA, C3C4均显著低于正常对照组(P<0.05);IgM高于正常对照组,差异无统计学意义(P>0.05);IgG显著高于正常对照组(P0.05)CU患者UAS评分与血清C3/C4浓度呈负相关(P<0.05),与血清IgAIgMIgG浓度不相关(P>0.05);生活质量评分与血清IgAIgMIgGC3C4浓度无显著相关性(P>0.05)。结论:血清IgAIgGC3C4浓度与慢性荨麻疹发病有关,其中血清C3/C4浓度或可成为慢性荨麻疹病情严重程度的评估指标。

Abstract:

Objective: To correlate the severity of chronic urticaria (CU) and the serum levels of immunoglobulins, C3 and C4. Methods: One hundred and sixty patients with chronic urticaria were enrolled in this study. Urticaria Activity Score (UAS) and Dermatology Life Quality Index (DLQI) score were used to assess the severity of the disease. The levels of serum IgA, IgM, IgG, C3 and C4 were detected and the results were compared with those in 80 controls. The correlation between the severity of the disease and the results of the tests was analyzed. Results: The level of serum IgA, C3, C4 in the patients with CU was significantly lower than that in the normal controls (P<0.05). The level of serum IgM was higher than that in the normal controls, but the difference was not significant (P>0.05). The level of serum IgG was significantly higher than that in normal control group (P<0.05). There was a linear negative correlation between UAS score and serum C3 and C4 levels, but the correlation with serum IgA, IgG and IgM was not significant (P>0.05). There was no significant linear correlation between DALI score with serum IgA, IgM, IgG, C3, C4 concentrations (P>0.05). Conclusion: The serum levels of IgA, IgG, C3, C4 may be related to the onset of CU and serum C3 and C4 concentrations can be used as an index of the severity of CU.
