伴有眼部症状的皮下结节型结节病一例

摘要： 结节病眼部患病率7%~60%不等，干眼症是结节病最常见的眼部症状，葡萄膜炎是眼结节病最常见的眼部受累形式。眼部症状可在出现全身症状前数月出现，也可作为孤立的症状出现，当患者出现眼部症状时，通常会忽略结节病的诊断。本文报道一例65岁女性患者。因双眼出现黑影1个月余就诊，右臀部可触及一1 cm×1 cm大深在皮下结节。眼部超声提示双眼葡萄膜炎，肺部影像学表现为特征性的肺部结节及纵隔内淋巴结肿大。右臀部皮下结节组织病理示真皮内可见多个由上皮样组织细胞组成的裸结节。

Abstract: The ocular prevalence of sarcoidosis ranges from 7% to 60%. Dry eye is the most common ocular symptom of sarcoidosis. Uveitis is the most common ocular involvement form of sarcoidosis. Ocular symptoms can occur months before the onset of systemic symptoms or as isolated symptoms, and the diagnosis of sarcoidosis is often ignored when patients only have ocular symptoms. Herein, we report a 65-year-old female patient was admitted to the hospital due to black shadows in both eyes for more than 1 month. The patient was found to have a subcutaneous nodule with a depth of 1 cm×1 cm on her right buttock. Ocular ultrasound indicated binocular uveitis, and lung imaging findings showed characteristic pulmonary nodules and mediastinal lymph node enlargement.  The histopathology of subcutaneous nodules of the right hip showed multiple nude nodules composed of epithelioid histopathocytes in the dermis.
