医防融合助力构建专科医院传染病防控模式——以某三甲皮肤病专科医院为例

Integration of medical services and prevention facilitates the construction of an infectious disease prevention and control model in specialized hospitals: a case study of a tertiary dermatology hospital

摘要: 目的:探索皮肤病专科医院医防融合模式,为提升专科医院公共卫生服务能力提供理论与实践依据。方法:收集华东地区某三甲皮肤病专科医院2018—2025年法定传染病报告数据、感染性皮肤病防治及医防融合相关资料,通过描述性流行病学方法、案例分析及焦点小组讨论,评价医防融合措施实施效果,总结专科医院医防融合模式。结果:2018—2025年该院门诊量增长0.75倍,累计报告法定传染病6062例,年报告例数增长3.72倍,以2022年医防融合强化为节点呈阶段性快速增长特征。报告病种从9种扩展至12种,新增猴痘、皮肤炭疽等新发再发传染病。尖锐湿疣、水痘、梅毒等6病种占报告总数的96.30%,凸显专科特色。同时成功处置多起海分枝杆菌、疥疮聚集性疫情。该院通过强化全链条的医防融合实践,有效提升了传染病监测敏感性和应急处置能力。结论:医防融合是破解皮肤病专科医院“医防割裂”困境的有效路径,其构建的“顶层设计-体系建设-能力保障-流程优化-科研支撑”模式,可为专科医疗机构传染病防控提供实践参考。

Abstract: Objective: To explore the integration model of medical services and prevention in specialized dermatology hospitals, providing a theoretical and practical basis for enhancing the public health service capabilities of specialized hospitals. Methods: Data on statutory infectious disease reporting, prevention and control of infectious skin diseases, and relevant materials on the integration of medical services and prevention were collected from a tertiary Grade A dermatology hospital in East China from 2018 to 2025. Descriptive epidemiological methods, case analysis, and focus group discussions were employed to evaluate the effectiveness of the implemented integration measures and to summarize the model. Results: From 2018 to 2025, the hospital's outpatient volume increased by 0.75 times, with a cumulative total of 6,062 statutory infectious disease cases reported. The annual number of reported cases grew by 3.72 times, showing a phased rapid growth characteristic with the reinforcement of the integration measures in 2022 as a turning point. The spectrum of reported diseases expanded from 9 to 12 types, including emerging and re-emerging infectious diseases such as monkeypox and cutaneous anthrax. Six diseases, including condyloma acuminatum, varicella, and syphilis, accounted for 96.30% of the total reports, highlighting the specialty characteristics of the hospital. Meanwhile, several clustered outbreaks of Mycobacterium marinum and scabies were successfully managed. Through strengthening the full-chain practice of medical-preventive integration, the hospital effectively improved its infectious disease surveillance sensitivity and emergency response capabilities. Conclusion: The integration of medical services and prevention is an effective pathway to resolve the dilemma of “separation between clinical care and prevention” in specialized dermatology hospitals. The established model comprising “top-level design, system construction, capacity assurance, process optimization, and research support” can provide a practical reference for infectious disease prevention and control in specialized medical institutions.
