误诊为特应性皮炎的卡波西肉瘤一例

摘要： 本文报道一例87岁男性患者，四肢红斑、丘疹、脱屑，伴明显瘙痒1年余，HIV阴性，初诊为特应性皮炎。行常规治疗后症状反复，二次组织病理特点结合免疫组化HHV8、CD34、CD31等指标阳性，最终确诊为经典型卡波西肉瘤。患者初诊时卡波西肉瘤皮损表现为湿疹样皮损，加之早期病理特征不典型、活检部位选择偏差导致误诊，该病例提示临床接诊老年顽固性、进展性湿疹样皮损时，需警惕肿瘤性病变。

Abstract: An 87-year-old male patient who presented with erythema, papules and scaling on the extremities accompanied by severe pruritus for more than one year.HIV was negative, and the initially diagnosed with atopic dermatitis. Conventional treatment only yielded recurrent symptoms. Re-examined histopathology and positive immunohistochemical markers (HHV8, CD34, CD31) confirmed classic Kaposi's sarcoma. The lesion presented eczematous features at initial visit. Atypical early pathological changes and improper biopsy site led to misdiagnosis of Kaposi's sarcoma.This case suggests that clinicians should be alert to neoplastic diseases when encountering elderly patients with intractable and progressive eczematous skin lesions.
