机械应力微创伤在银屑病关节炎发病机制中的研究进展

摘要: 银屑病关节炎(psoriatic arthritis,PsA) 严重可出现关节畸形,影响患者日常生活。近年来,国内外对银屑病关节炎发病机制的研究取得了较大进展,特别是关节局部微创伤后引起的深部同形反应受到关注。滑膜-附着点复合体在银屑病关节炎机制研究中扮演重要角色,在创伤及机械应力作用下,滑膜-附着点复合体部位可以发生深部同形反应,激活IL-23/Th17轴的先天性免疫反应,同适应性免疫反应相互作用,导致PsA发生。本文对近年来国内外机械应力相关微创伤在PsA的发生发展机制中的研究进展进行了综述。

Abstract: Psoriatic arthritis(PsA) can lead to joint deformities in severe cases, affecting patients' daily life. In recent years, domestical and international progress has been made in the pathogenesis of PsA, especially focus on the deep koebner phenomenon triggered by local microtrauma to the joints. The Synovio-Entheseal Complex (SEC) plays an important role in the mechanistic studies of PsA. Due to trauma or mechanical stress, the deep Koebner response occurred at the SEC, activating the innate immune response through the IL-23/Th17 axis, which interacts with the adaptive immune responses, ultimately leading to the development of PsA. This paper reviews the update of mechanical stress related microtrauma in the pathogenesis and development of PsA in recent years both domestically and internationally.
