三亚市1157例浅部真菌感染病原菌及药物敏感性分析

Molecular biological identification and drug sensitivity analysis of pathogenic fungi in shallow fungal infections in Sanya City

摘要: 目的:明确我院浅部真菌感染患者真菌种类及抗真菌药物的敏感性。方法:收集我院2019年7月至2021年12月浅部真菌感染患者,进行镜检、培养菌株分离,分子生物学鉴定。癣菌按照美国临床和实验室标准化委员会的M38-A2方案,检测受试菌株对7种常见抗真菌药物的敏感性;酵母菌使用药敏试剂盒,检测受试菌株对5种常见抗真菌药物的敏感性。结果:2143例疑似浅部真菌感染患者中1157例镜检阳性,并对分离的33株癣菌和23株念珠菌进行分子生物学鉴定,结果示33株癣菌中,红色毛癣菌为优势菌种(27,81.8%);23株念珠菌中,近平滑念珠菌和白色念珠菌占比较多,分别为34.78%、26.09%。21株念珠菌药敏显示两性霉素B的敏感率最高;22株红色毛癣菌药物的敏感性结果显示,环吡酮,灰黄霉素、伊曲康唑、伏立康唑、氟康唑、特比萘芬、泊沙康唑MIC值均处于较低水平,以伏立康唑平均MIC值最低,为0.0096 mg/L。结论:浅部感染病例主要为红色毛癣菌、近平滑念珠菌和白色念珠菌。念珠菌对两性霉素B敏感,红色毛癣菌对伏立康唑敏感。

Abstract: Objective: To identify the fungal species of patients with superficial fungal infections in our hospital and the sensitivity to antifungal drugs. Methods: Patients with superficial fungal infections in our hospital from July 2019 to December 2021 were collected for microscopic examination, culture strain isolation, and molecular biological identification. For dermatophytes, the sensitivity of the tested strains to 7 common antifungal drugs was detected according to the M38-A2 protocol of the Clinical and Laboratory Standards Institute (CLSI) in the United States; for yeasts, a drug sensitivity test kit was used to detect the sensitivity of the tested strains to 5 common antifungal drugs. Results: Among 2143 suspected patients with superficial fungal infections, 1157 cases were positive by microscopic examination. Among them, performed on 33 isolated dermatophyte strains and 23 Candida strains. The results showed that among the 33 dermatophyte strains, Trichophyton rubrum was the dominant species (27, 81.8%); among the 23 Candida strains, Candida parapsilosis and Candida albicans were the most, accounted for 34.78% and 26.09% respectively. The drug sensitivity test of 21 Candida strains showed that amphotericin B had the highest sensitivity; the sensitivity results of 22 Trichophyton rubrum strains showed that the MIC of ciclopirox, griseofulvin, itraconazole, voriconazole, fluconazole, terbinafine, and posaconazole were all at low levels, and the geometric mean MIC of voriconazole was the lowest (0.0096 mg/L). Conclusion: The main pathogens of superficial infections are Trichophyton rubrum, Candida parapsilosis, and Candida albicans. Candida is sensitive to amphotericin B, and Trichophyton rubrum is sensitive to voriconazole.
