Abstract: A case of papuloid acantholysis of genitalia is reported. A 30-year-old male presented with gray white papules around the anus with partial fusion and pruritus for two years. Laboratory examination showed positive for HPV6 and negative for acetic acid white test, Treponema pallidum gelatin agglutination test (TPPA) and HIV. Histopathological examination showed cleft between the basal layer and the spinous layer with acantholytic dyskeratotic cells and lymphocyte infiltration. Direct immunofluorescence: IgG, IgM, IgA and C3 were negative. After 6 months of topical application of 0.03% tacrolimus, the papules subside and the itching was relieved.
