Abstract: A 64-year-old woman presented with 20 days history of nodules and dark red plaques with pain involved in both lower limbs. Histology revealed the fat layer were the detoxification cells, more eosinophilic granulocytes and a little neutrophilsand lymphatic infiltration. PET-CT showed:pancreatic tail space, consider pancreatic (primary lesion) malignant tumor; multiple metastatic tumors in the liver; possibility of lnguinal lymph node metastasis. Liver biopsy tissue pathology suggested metastatic adenocarcinoma from the digestive system. The diagnosis of pancreatic lipomyelitis (pancreatic cancer with liver metastasis) was made, and the patient is being treated with chemotherapy. For patients with painful nodules and plaques of lower limbs, detailed medical history should be inquired, relevant imaging examination and skin histopathological examination should be made to avoid missed diagnosis and misdiagnosis of pancreatic pannitis.
