12例重症麻风反应患者临床分析

摘要: 目的:总结重症麻风反应患者的临床特征、治疗及转归。方法:采用描述性统计学方法对我院2022年1月至2023年12月收治的12例重症麻风反应患者的临床资料进行回顾性分析。结果:12例患者,男8例,女4例,平均年龄36.08岁,均为中间界线类偏瘤型麻风,其中Ⅰ型麻风反应2例,Ⅱ型麻风反应6例,混合型麻风反应4例。11例伴有神经炎表现(91.67%),发生2级畸残4例,1级畸残3例,畸残比为58.33%。所有患者均给予糖皮质激素治疗,部分联合沙利度胺或其他免疫抑制剂,12例患者麻风反应均消退,4例患者(33.33%)出院后麻风反应加重或再发。结论:重症麻风反应病情复杂易复发,神经受累常见,糖皮质激素是主要治疗药物。

Abstract: Objective: To summarize the clinical features, treatment and outcome of patients with severe leprosy reaction retrospectively. Methods: Descriptive statistics were used to analyze the clinical data of 12 patients with severe leprosy reaction admitted to our hospital from January 2022 to December 2023 retrospectively. Results: Among the 12 patients, there were 8 males and 4 females with an average age of 36.08 years. All cases were borderline lepromatous, including 2 cases of type I leprosy, 6 cases of type II leprosy and 4 cases of mixed leprosy. 11 cases were accompanied by neuritis (91.67%), 4 cases had grade 2 deformity, 3 cases had grade 1 deformity, and the deformity ratio was 58.33%. All patients were treated with glucocorticoid, some with thalidomide or other immunosuppressants, and the leprosy response subsided in all 12 patients. The leprosy reaction worsened or recurred in 4 patients (33.33%) after discharge. Conclusion: The symptoms of severe leprosy reactions present complex and are prone to recurrence, nerve involvement is common, glucocorticoid is the main treatment.
