干燥综合征合并化脓性腮腺炎及非结核分枝杆菌感染一例

摘要： 本文报道一例干燥综合征(Sj?gren's sydrome,SS)合并化脓性腮腺炎，并伴有非结核分枝杆菌(nontuberculosis mycobacteria,NTM)感染的罕见病例。患者为中年女性，有6年SS及慢性复发性腮腺炎病史，此次因化脓性腮腺炎行常规抗生素治疗无效，症状加重入院，患者病灶脓液检测到NTM。根据NTM临床诊治指南及菌种药敏试验结果给予克拉霉素，联合阿米卡星及莫西沙星抗感染治疗。腮腺腺体病灶局限，行左侧腮腺区脓肿切开引流术。治疗2周后创面愈合，腮腺肿胀完全吸收，颜面部恢复正常。

Abstract: We herein report a rare case of Sj?gren's syndrome (SS) complicated with suppurative parotitis and nontuberculous mycobacterial (NTM) infection. The patient was a middle-aged female with a 6-year history of SS and chronic recurrent parotitis. She was admitted due to suppurative parotitis that was unresponsive to conventional antibiotic therapy. NTM was detected in the pus from the lesion. According to the clinical guidelines for the diagnosis and treatment of NTM infection and the results of drug susceptibility testing, anti-infective therapy with clarithromycin combined with amikacin and moxifloxacin was administered. Since the parotid lesion was localized, incision and drainage of the left parotid abscess was performed. After 2 weeks of continuous treatment, the wound healed, parotid swelling resolved completely, and her facial appearance returned to normal.
