宿迁市2005—2025年新发麻风病例流行病学特征分析

Epidemiological analysis of newly detected leprosy cases in Suqian City from 2005 to 2025

摘要: 目的:了解2005—2025年宿迁市新发麻风病例的发现方式及分布情况,分析变化趋势,为今后开展麻风防治工作提供科学依据。方法:通过全国麻风防治管理信息系统,收集宿迁市2005—2025年新发麻风病例的详细资料,采用描述流行病学方法对新发麻风病例进行分析。结果:2005—2025年宿迁市累计发现61例新发麻风病例,年均发现率为0.06/105,麻风发现率呈下降趋势(χ2趋势=17.05,P<0.001),发现率最高的年份为2010年(0.30/105)。新发现病例平均确诊年龄为(47.4±16.4)岁,以男性、农民为主。发现方式以皮肤科就诊发现为主(33例,54.10%)。从发病到确诊的平均延迟期为57.68个月,其中≥12个月49例。2级畸残率为24.59%,其中诊断延迟期≥12个月畸残率为51.02%,<12个月为8.33%。Logistic回归分析显示诊断延迟期≥12个月的病例发生畸残的风险是诊断延迟期<12个月的11.2倍。结论:宿迁市2005-2025年麻风的发现率和患病率呈下降趋势,麻风处于低流行状态。新发病例主要通过皮肤科门诊就诊发现,存在诊断延迟、2级畸残率较高的问题。

Abstract: Objective: To investigate the detection methods and distribution patterns of newly identified leprosy cases in Suqian City from 2005 to 2025, analyze trends, and provide scientific basis for future leprosy prevention and control efforts. Methods: Detailed data on newly diagnosed leprosy cases in Suqian City from 2005 to 2025 were collected through the National Leprosy Prevention and Control Management Information System. Descriptive epidemiological methods were employed to analyze these cases. Results: A total of 61 new leprosy cases were detected in Suqian City from 2005 to 2025, with an average annual detection rate of 0.06/105. The leprosy detection rate showed a declining trend (trend χ2=17.05, P<0.001), and the highest annual rate was recorded in 2010 (0.30/105). The mean age at definitive diagnosis of newly detected cases was (47.4±16.4) years, with males and farmers constituting the predominant subgroups. Thirty-three cases (54.10%) were identified through outpatient consultation in the dermatology department, which was the primary detection approach. The average diagnostic delay from symptom onset to confirmed diagnosis was 57.68 months, and 49 cases experienced a diagnostic delay of no less than 12 months. The overall grade 2 disability rate was 24.59%; specifically, the disability rate reached 51.02% in patients with a diagnostic delay ≥12 months, compared with 8.33% in those with a delay shorter than 12 months. Logistic regression analysis indicated that patients with a diagnostic delay of 12 months or longer had an 11.2-fold higher risk of developing disability. Conclusion: The detection rate and prevalence rate of leprosy in Suqian City both declined from 2005 to 2025, indicating a low endemic status of leprosy locally. Most new cases were identified during dermatology outpatient visits, accompanied by prominent problems including diagnostic delay and a relatively high grade 2 disability rate.
