夫那奇珠单抗治疗毛囊闭锁三联征三例

Three cases of follicular occlusion triad treated with vunakizumab

摘要: 毛囊闭锁三联征(follicular occlusion triad,FOT)是一组以毛囊角化过度、闭塞及慢性复发性炎症为特征的疾病,传统治疗疗效欠佳。IL-17A是驱动慢性炎症的核心细胞因子之一,夫那奇珠单抗可选择性与IL-17A亚基结合并中和其生物活性。迄今为止,尚未见其用于治疗FOT的报道。本文报道3例经传统治疗无效的FOT患者,治疗前筛查未见活动性感染或生物制剂使用禁忌,予夫那奇珠单抗治疗4个月后疼痛消失,皮损消退,窦道闭合,炎症指标复常,其中2例达到化脓性汗腺炎临床应答HiSCR60、1例达HiSCR70,IHS4评分皆显著下降,均未见明显不良反应。本文病例表明,夫那奇珠单抗对传统治疗无效的毛囊闭锁三联征患者显示出良好疗效和安全性。

Abstract: Follicular occlusion triad (FOT) is a spectrum of disorders characterized by hyperkeratosis and occlusion of hair follicles accompanied by chronic recurrent inflammation, which responds poorly to conventional therapies. Interleukin-17A (IL-17A) is one of the key cytokines driving chronic inflammatory responses. Vunakizumab selectively binds to the IL-17A subunit and neutralizes its biological activity. To date, there have been no published reports regarding its application in the treatment of FOT. We report three patients with FOT who were refractory to conventional therapies. Baseline screening revealed no active infection or contraindications to biologic therapy. After 4 months of treatment, all patients experienced complete resolution of pain, marked regression of skin lesions, closure of sinus tracts, and normalization of inflammatory markers. Two patients achieved HiSCR60 and one achieved HiSCR70, accompanied by a substantial reduction in International Hidradenitis Suppurativa Severity Score System (IHS4) scores. No obvious adverse events were observed. These cases suggest that vunakizumab may have favorable short-term efficacy and safety in patients with follicular occlusion triad refractory to conventional treatment.
