银屑病合并2型糖尿病相关因素分析

The related factors of psoriasis complicated with type 2 diabetes

摘要: 目的:分析银屑病患者合并2型糖尿病的相关因素。方法:以2008-2016年在山东省皮肤病医院住院治疗银屑病患者为病例1组,以体检中心查体者为对照1组,分别比较两组之间性别、年龄、空腹血糖数值差异。将病例1组根据是否合并2型糖尿病划分为病例2组和对照2组,比较发病年龄、住院次数、住院天数及入院时皮损受累面积、PASI评分等。结果:病例1组患者570例中,164例合并2型糖尿病(28.77%),对照1组健康查体者4513例中,405例合并2型糖尿病(8.97%),2型糖尿病患病率差异有统计学意义(P<0.05)。病例1组男女银屑病患者合并2型糖尿病的患病率为分别为31.5%和19.2%,差异具有统计学意义(P<0.05)。病例2组患者164例,对照2组患者406例,病例2组患者较对照2组银屑病发病年龄低,病程长,住院频率高。病例2组中吸烟患者43.9%高于对照2组的28.3%(P<0.05)。皮损面积和PASI评分在病例2组与对照2组间比较,差异均无统计学意义(Ps>0.05)。结论:银屑病患者的2型糖尿病患病率高于健康人群。银屑病患者的性别、发病年龄和吸烟史可能与银屑病合并2型糖尿病有关。

Abstract: Objective: To analyze the related factors of psoriasis complicated with type 2 diabetes. Methods: The inpatients with psoriasis in Shandong Provincial Hospital for Skin Diseases from 2008 to 2016 were collected and as the case group 1, and healthy physical examinees were as the control group 1. Compare the differences in gender, age, and fasting blood glucose values between the two groups. Then the patients in the case group 1 were divided into the case group 2 and the control group 2 according to whether with type 2 diabetes or not. The information of gender, hospitalization frequency and days, body surface area of lesions, PASI score were compared. Results: There were 570 patients in the case group 1 including 164 (28.77%) with type 2 diabetes and 4513 controls in the control group 1 including 164 (28.77%) with type 2 diabetes. In the case group 1, the morbidity of the male and female patients with type 2 diabetes was 31.5% and 19.2%, with a significant difference (P<0.05). There were 164 and 406 cases in the case group 2 and control group 2. Compared to the control group 2, the onset age was younger, the course was longer and the hospitalization frequency was more in the case group 2, with significant differences (Ps<0.05). The proportion of smokers in the case group 2 was 43.9%, which was higher than those in the control group 2, with a significant difference (P<0.05). There was no significant difference of body surface area of lesions and PASI score between the case group 2 and the control group 2 (Ps>0.05).  Conclusion: The incidence of type 2 diabetes in the patients of psoriasis is higher than that in healthy controls. The gender, age and smoking history may be related to the psoriasis complicated with type 2 diabetes.
