皮肤转移癌62例临床及组织病理分析

摘要: 回顾性分析我院皮肤科2010年1月至2021年1月期间62例皮肤转移癌患者的临床及组织病理特征。结果示62例患者中,男34例(54.8%),女28例(45.2%),发病平均年龄为(59.2±12.5)岁(41~89岁)。患者就诊时间中位数为2个月。45例患者仅有一个转移灶,17例有两个或以上转移灶。好发部位依次为胸部(18例),头部(14例),四肢(12例)。皮损表现多为结节、包块(49例),其次红斑(9例)、丘疹(4例)。原发肿瘤中最多的为转移性腺癌56例(90.3%),其次为转移性鳞癌4例(6.5%),另2例为小细胞癌。男性以肺癌最常见,女性以乳腺癌最多见。62例病例中19例误诊为其他疾病,首诊误诊率为30.6%。皮肤转移癌临床上容易误诊,应提高对皮肤转移癌的警惕性。

Abstract: The data of 62 patients with cutaneous metastatic carcinoma in the dermatology department of our hospital from January 2010 to January 2021 were retrospectively analyzed. Among the 62 patients, 34 cases were male (54.8%) and 28 cases were female (45.2%).The mean age of onset was 59.2±12.5 years old (41~89 years old). The median patient visit time was 2 months. There were 45 cases (72.6%) with only one metastatic lesion, and 17 (27.4%) with two or more metastases. The most common sites were chest (18 cases), head (14 cases), and limbs (12 cases). Most of the lesions were nodules and masses (49 cases), followed by erythema (9 cases) and papules (4 cases). Among the primary tumors, 56 cases (90.3%) were metastatic adenocarcinoma, 4 (6.5%) were metastatic squamous cell carcinoma, and 2 were small cell carcinoma. Lung cancer was the most common tumor in males, and breast cancer was the most common tumor in females. Nineteen of the 62 cases were misdiagnosed as other diseases, and the misdiagnosis rate of the first diagnosis was 30.6%. Cutaneous metastatic carcinoma is easy to be misdiagnosed clinically and vigilance should be raised.
