头皮原发性皮肤腺样囊性癌一例并文献复习

Primary cutaneous adenoid cystic carcinoma on the scalp: a case report and related literature review

摘要： 报道头皮原发性皮肤腺样囊性癌一例并对相关文献进行复习。患者，女，44岁。头顶部结节10年，伴疼痛1年。皮损表现为头顶部单发，直径1.5 cm粉红色结节，无破溃。组织病理示：肿瘤位于真皮内，团块状瘤团聚集分布，瘤团中央有较多管状和囊腔状结构，囊腔内可见黏蛋白沉积，形成筛状结构，瘤细胞为单一嗜碱性细胞，胞核致密，核仁不明显，胞质淡染，核分裂像散在见。免疫组化示：CD117(+)；CK7(+);CK5/6(+)；S-100(局灶+)；P63(+)；SOX-10(+)；GCDFP15(-)；Ki-67阳性细胞约占20%。行外科手术切除，随访两年未复发。

Abstract: A case of primary cutaneous adenoid cystic carcinoma ( PACC) of the scalp was reported and the related literature were reviewed. A  44-year-old female, presented, nodules on the overhead for 10 years with pain for 1 year. Histopathological examination showed the tumor  was located in the dermis with clumpy clumps clustered and distributed. There were many tubular and cystoid structures in the center of the clumps, and mucin deposits were visible in the cystoid, forming a sieve structure. The tumor cells were single basophil cells with dense nuclei, not obvious nucleoli, light cytoplasm, and mitotic astigosis. Immunohistochemistry results showed CD117 (+), CK7 (+), CK5/6 (+), S-100 (focal +), P63 (+), SOX-10 (+), GCDFP15 (-),  Ki-67 positive cells accounted for about 20%. Surgical resection was performed with no recurrence after two years of follow-up.
