TNF-α抑制剂治疗头部脓肿性穿掘性毛囊周围炎一例并文献复习

Treatment of perifolliculitis capitis abscedens et suffodiens with TNF-α inhibitors: a case report and literature review

摘要： 患者，男，45岁。头皮丘疹、结节、脓肿、破溃反复，伴疼痛5年。诊断为头部脓肿性穿掘性毛囊周围炎。经常规治疗效果欠佳，改用阿达木单抗治疗，治疗4个半月后，疼痛显著减轻、结节缩小、炎症缓解、脓液分泌减少/停止。患者继续治疗并随访中。已有文献报道了应用TNF-α抑制剂治疗PCAS的临床疗效。本文对本例患者及既往文献进行了回顾性分析。

Abstract: A 45-year-old male presented with scalp papules, nodules, abscesses, and recurrent ulceration accompanied by pain for 5 years. He was diagnosed with perifolliculitis capitis abscedens et suffodiens. After an unsatisfactory response to conventional treatment, the patient was treated with adalimumab. After four and a half months of treatment, significant alleviation of pain, shrinkage of nodules, resolution of inflammation, and reduction or cessation of purulent discharge were observed. The patient is currently undergoing continuous treatment and follow-up. Some literature has reported on the clinical efficacy of TNF-α inhibitors in treating PCAS. This paper conducts a retrospective analysis of the treatment of the present case and previous literature.
