原发皮肤结外NK/T细胞淋巴瘤三例临床分析

Clinical analysis of primary cutaneous extranodal NK/T-cell lymphoma in 3 cases

摘要: 原发皮肤结外NK/T细胞淋巴瘤临床表现多样,疾病进展快,预后不良,治愈率低。本文报道3例原发皮肤结外NK/T细胞淋巴瘤病例,其中男1例,女2例,皮损分别位于左外踝部、左上肢及右上眼睑,皮损表现为红斑、水疱、结节、糜烂、溃疡等病变。HE染色均显示真皮层大量弥漫性肿瘤细胞浸润;原位杂交示肿瘤细胞EB病毒编码RNA(EBER)阳性;免疫组化表明肿瘤细胞均阳性表达CD3、CD30、Ki-67(50%),患者2和3 CD56阳性,患者1 CD56阴性。患者1经化疗联合放疗后好转,但在治疗过程中出现颅内出血并发症;患者2化疗后继发肺部感染死亡,患者3化疗效果不佳后失访。

Abstract: Primary cutaneous extranodal NK/T-cell lymphoma (PC-ENKTL) exhibits a variety of clinical manifestations, rapid disease progression, poor prognosis, and low cure rates. We report three cases of primary cutaneous extranodal NK/T-cell lymphoma, including one male patient and two female patients, with skin lesions located on the left outer ankle, left upper extremity, and right upper eyelid respectively. Skin lesions were characterized by erythema, blisters, nodules, erosions, ulcers and other lesions. Hematoxylin-eosin staining revealed extensive infiltration of tumor cells throughout the dermis in all cases. In situ hybridization revealed positive staining for EB virus encoded RNA (EBER) in all cases. Immunohistochemical examination showed positive for CD3, CD30, and Ki-67 (50%) in all cases, and positive for CD56 in cases 2 and 3. Case 1 showed improvement after chemotherapy combined with radiotherapy, but developed intracranial hemorrhage as a complication during treatment. Case 2 died from secondary lung infection after chemotherapy. Case 3 was lost to follow-up after having poor response to chemotherapy.
