中性粒细胞/淋巴细胞比值对伴脏器损害过敏性紫癜的临床意义

Clincial significance of neutrophil-to-lymphocyte ratio for Henoch-Schönlein purpura with viscera damage

摘要： 目的:检测中性粒细胞/淋巴细胞比值（NLR比值）、血沉、白细胞计数、血红蛋白、红细胞计数，以探讨其对诊断及预测伴脏器损害过敏性紫癜（HSP）的临床意义。方法: 检测32例单纯型HSP患者和44例伴有脏器损害的HSP患者的NLR比值等血液学指标。结果:（1）伴脏器损害HSP组的NLR比值较单纯型HSP组明显升高（P<0.05）；（2）根据ROC工作曲线下面积(AUC)评估显示，NLR的AUC为0.708；（3）HSP患者的NLR比值与白细胞计数呈显著正相关（r=0.486，P<0.05）。结论: NLR比值对于诊断和预测脏器损害型HSP预后有一定的临床价值。

Abstract: Objective: To detect the levels of neutrophil-to-lymphocyte ratio (NLR), erythrocyte sedimentation rate (ESR) and white blood cell count (WBC) to help diagnosis of Henoch-Sch?nlein purpura (HSP) with viscera damage. Methods: A total of 46 patients were collected, including 32 HSP of simplex type and 44 HSP with viscera damage. The levels of NLR, ESR, WBC were detected. Results: (1) NLR in the patients with HSP with viscera damage was significantly higher than that in the patients with simple HSP patients (P<0.05). (2) The areas under the receiver operating characteristic (ROC) curve of NLR for the diagnosis of HSP with viscera damage was 0.708 (P <0.05). (3) NLR was associated with the white blood cell count (r=0.486, P<0.001). Conclusion: NLR is a potential clinical indicator for diagnosis and prognosis for HSP with viscera damage.
