Abstract: Atrophying tinea versicolor (ATV) is a rare variant of tinea versicolor characterized by cutaneous atrophy with dyspigmentation. We report a 39-year-old male with 8-year history of ATV, initially misdiagnosed as common tinea versicolor, confirmed by pathology and fungal examination . The patient received oral itraconazole (100mg daily) combined with topical compound benzoic acid tincture and sertaconazole nitrate 2% cream. Complete resolution was achieved after one month of treatment, with no recurrence observed at one-month follow-up. A literature review of 32 patients showed that this disease predominantly affects young and middle-aged individuals (median age 35 years), with an equal gender ratio. 21.9% of cases were associated with long-term topical glucocorticoid use. The effective rate of antifungal treatment was 62.5%, while 37.5% of patients had persistent atrophy as a sequela.
