Abstract: Eccrine spiral adenocarcinoma is a very rare cutaneous malignant tumor with atypical clinical manifestations. A case of eccrine spiradenocarcinoma was reported. The patient was a 62-year-old female who presented with red nodules on the scalp for 10 years, ulceration, bleeding, and pain for 2 years. Histopathological examination showed the tumor was composed of a single basal-like cell pattern in a multilobulated pattern, with mildly atypical cells. Pathological mitotic figures were common, and spiral adenoma elements were seen in some areas. Immunohistochemical staining found extensive CK (+), P63 (+), CD5/6 (+), positive EMA(+) and CEA(+), CK7 (slightly +), Ki-67 index (local 15%). The diagnosis of eccrine spiradenocarcinoma was made. After extensive resection of the lesion, the patient recovered well, and no recurrence or metastasis was found during a follow-up of 30 months.
