蚰蜒叮咬致横纹肌溶解一例

摘要: 患者,男,19岁,左手被蚰蜒叮咬后出现红肿,伴剧烈疼痛,病情进展迅速,出现黑紫、大疱、糜烂、破溃,肌力下降,肌酸激酶明显增高(17 123 U/L),是正常值上限的55倍。诊断为蚰蜒皮炎并横纹肌溶解。给予甲泼尼龙琥珀酸钠80 mg/d及其他对症治疗。治疗11天后复查肌酶谱、肝功、尿常规均正常,肌力恢复正常。出院35天后随访,患者左手肿胀消退,少许色沉,功能恢复正常。

Abstract: A 19-year-old male patient developed redness and swelling on his left hand after being bitten by a centipede, accompanied by severe pain. The condition progressed rapidly, presenting with purplish-black discoloration, large blisters, erosion, and ulceration. There was a decrease in muscle strength, and the creatine kinase level was significantly elevated (17 123 U/L), which was 55 times the upper limit of the normal range. The diagnosis was centipede dermatitis complicated with rhabdomyolysis. He was given methylprednisolone sodium succinate 80 mg/day along with other symptomatic treatments. After 11 days of treatment, re-examinations showed that the muscle enzyme profile, liver function, and urine routine were all normal, and muscle strength returned to normal. During the follow-up 35 days after discharge, the swelling of the patient's left hand subsided, with a bit of skin pigmentation remaining, and the hand function returned to normal.
