SAPHO综合征前胸壁超声特征分析

Sonographic characteristics of SAPHO syndrome involving the anterior chest wall

摘要: 目的:明确SAPHO综合征患者前胸壁的超声特征。方法:回顾性分析2021-2025年确诊的SAPHO综合征且前胸壁受累患者的超声检查资料。结果:共有110例患者被纳入本次研究,前胸壁病变呈不对称分布,其主要累及胸锁关节、胸肋关节及胸骨柄区域。55例患者检测到胸锁关节炎, 30 例胸肋关节受累。骨质侵蚀的分布呈现以下特征:胸骨是最常见的受累部位(75例,68.1%),其次为肋骨骨质受累(43例,39.1%)和锁骨骨质破坏(39例,35.6%)。此外,42例(38.2%)患者伴有邻近软组织增厚或回声改变等影像学表现。结论:SAPHO综合征的前胸壁病变表现为滑膜炎、骨质侵蚀、软组织肿胀,为其早期诊断和病情评估提供重要依据。

Abstract: Objective: To explore the ultrasonographic features of the anterior chest wall(ACW)in patients with SAPHO syndrome. Methods: he ultrasonic examination data of patients with SAPHO syndrome confirmed from 2021 to 2025 and with anterior chest wall involvement were retrospectively analyzed. Results: A total of 110 patients were included in this study. The lesions of the anterior chest wall showed an asymmetric distribution, mainly involving the sternoclavicular joint, sternocostal joint and manubrium sterni region. Sternoclavicular arthritis was detected in 55 patients, and 30 patients had sternocostal joint involvement. The distribution of bone erosion showed the following characteristics: the sternum was the most common involved site (75 cases, 68.1%), followed by rib bone involvement (43 cases, 39.1%) and clavicle bone destruction (39 cases, 35.6%). In addition, 42 patients (38.2%) were accompanied by imaging findings such as adjacent soft tissue thickening or echo changes. Conclusion: The anterior chest wall lesions of SAPHO syndrome are characterized by synovitis, bone erosion and soft tissue swelling, which provide an important basis for its early diagnosis and condition assessment.
