中重度银屑病与银屑病性关节炎肌骨超声关节改变的比较

摘要: 目的:比较中重度银屑病和银屑病性关节炎患者的关节超声表现的差异。方法:纳入2014年2月至2017年8月住院期间接受肌骨超声检查的中重度银屑病和银屑病性关节炎患者。评估两组患者的附着点炎、滑膜炎、骨侵蚀、关节间隙狭窄、甲病变等关节相关病变。结果:与310例中重度银屑病患者相比,112例银屑病性关节炎患者的年龄更大、病程更长、关节超声异常表现比例更高。96.4%(108/112)的银屑病性关节炎患者和81.9%(254/310)的中重度银屑病患者超声显示关节改变。结论:本研究中重度银屑病患者和银屑病性关节炎患者关节超声异常比较均较高,临床上应重视中重度银屑病患者的关节改变筛查。

Abstract: Objective: To compare the characteristics of arthropathy demonstrated by musculoskeletal ultrasound imaging between the patients with moderate-to-severe psoriasis and the patients with psoriatic arthritis. Methods: The data of the patients with moderate-to-severe psoriasis and psoriatic arthritis (PsA) underwent musculoskeletal ultrasound examination from February 2014 to August 2017 was analyzed retrospectively. The frequency and distribution of enthesitis, synovitis, bone erosion, narrowing of joint space, nail lesions were assessed and compared between the two groups. Results: Compared to 310 moderate-to-severe psoriasis patients, the age was older, the duration of the disease was longer, and the abnormal ultrasonic manifestations of joint pain was more common in 110 psoriatic arthritis patients. Ultrasound imaging showed that 108 (96.4%) patients with PsA and 254 (81.9%) moderate-to-severe psoriasis patients had joint lesions. Conclusion: The frequency of the abnormal ultrasonic manifescations of the joint are high in both patients with moderate-to-severe psoriasis and PsA in this study and the joint examination should be attracted attention in those patients.
