PICC置管术后透明敷料贴敷致接触性皮炎资料分析

摘要: 目的:明确972例恶性肿瘤患者置入PICC置管术后因透明敷料所致接触性皮炎的发生率和危险因素。方法:对恶性肿瘤置入PICC导管后因透明敷料所致接触性皮炎患者的性别、年龄、体重指数、cTNM分期、药物过敏史、肿瘤类型、嗜酸性粒细胞和嗜碱性粒细胞水平等可能影响接触性皮炎发生的危险因素进行单因素和多因素分析。结果:972例置入PICC导管肿瘤患者中,有80例接触性皮炎(男21例,女59例),发生率为8.23%;单因素分析显示,嗜酸性粒细胞水平、睡眠状态、体重指数、药物过敏史与接触性皮炎发生有关(P<0.05);多因素分析显示,失眠、肥胖、药物过敏史是接触性皮炎的独立危险因素(P<0.001)。结论:PICC置管术后因透明敷料所致的相关接触性皮炎的发生率较高。失眠、肥胖、药物过敏史是PICC置管术后透明敷料所致接触性皮炎的独立危险因素。

Abstract: Objective: To determine the incidence and risk factors of contact dermatitis caused by transparent dressing after peripherally inserted central catheters (PICCs) catheterization in 972 patients with malignant tumor. Methods: The risk factors of contact dermatitis were analyzed by univariate and multivariate logistic regression analysis, including gender, age, body mass index, cTNM stage, history of drug allergy, tumor type, and the levels of eosinophils and basophils in peripheral blood. Results: Of 972 patients, 80 patients (21 males and 59 females) suffered from contact dermatitis caused by transparent dressing after PICCs. The incidence of contact dermatitis was 8.23%. Univariate analysis showed that eosinophil count, sleep status, body mass index and drug allergy history were associated with contact dermatitis (P<0.05). Multivariate logistic regression analysis showed that insomnia, obesity and drug allergy were independent risk factors for contact dermatitis(P<0.001). Conclusion: The incidence of contact dermatitis caused by transparent dressings is high. Insomnia, obesity and drug allergy history are the independent risk factors for the contact dermatitis caused by transparent dressings after PICCs.
