皮肤镜辅助诊断甲下鳞癌一例并文献复习

摘要： 甲下鳞癌的早期诊断较为困难，采用无创性的检查方法可为早期诊断提供较大帮助，相关文献报道甲下鳞癌皮肤镜下可见鳞屑、痂皮或角质，点状、发夹状弯曲血管和线状不规则血管，白色晕圈，并提到白色圈环可以作为鳞状细胞癌的特异性皮肤镜表现。本文患者右手无名指指甲下增生物1年，皮肤镜下可见线状不规则血管结构，边缘模糊，甲下角化过度、局部溃疡、血痂，结合临床初步诊断为甲下鳞状细胞癌，经组织病理明确诊断后行皮肤肿瘤扩大切除术。术后2个月患者手指创面愈合良好，手指活动无受限。

Abstract: The early diagnosis of subungual squamous cell carcinoma are rather difficult. The non-invasive examination methods can offer significant assistance to early diagnosis. Relevant literature has reported that under dermatoscopy, scales, crusts, keratin, punctate vessels, hairpin-shaped curved blood vessels and linear irregular blood vessels, white circles could be seen, and the white circles can be regarded as a specific dermatoscopic manifestation of squamous cell carcinoma. This patient presented with a neoplasm under the nail of the right ring finger for 1 year. Under dermoscopy, linear and irregular vascular structures can be seen, with blurred edges, excessive keratinization, local ulcers, and blood scabs. A preliminary diagnosis of subungual squamous cell carcinoma was made based on the clinical manifestations and dermoscopy findings. The diagnosis was identified through histopathology, then an expanded resection of the skin tumor were performed. The finger wound of the patient healed well 2 months after surgery, and there is no restriction on the movement of the finger.
