Abstract: Orf is a zoonotic disease caused by the orf virus. Typical orf presents with skin papules, vesicles, pustules, and crusts, whereas pseudo-vesicular orf is clinically rare. In this case, a 58-year-old male patient had a history of contact with orf-infected sheep. Physical examination revealed an edematous erythema on the index finger of the right hand and the right wrist, with multiple pseudo-vesicles on the surface and a central pustule. Histopathological examination of the skin lesion showed focal spongiosis, reticular degeneration, and vesicle formation in the epidermis; edema in the superficial dermis, as well as dense perivascular infiltrates dominated by mononuclear cells in the dermis and subcutaneous tissue, with scattered eosinophils. The diagnosis of orf was confirmed. The skin lesions gradually resolved following antiviral and symptomatic treatment.
