1990-2023年中国儿童疥疮疾病负担研究

Analysis of the disease burden of scabies among Chinese children from 1990 to 2023

摘要: 目的:分析1990-2023年中国儿童疥疮疾病负担及变化趋势,并为其防控提供参考依据。方法:从2023年的全球疾病负担(global burden of disease,GBD)数据库中提取1990-2023年中国儿童疥疮数据,采用患病、发病、伤残调整寿命年(disability-adjusted life years, DALYs)及年度变化百分比(annual percentage change, APC)和平均年度变化百分比(average annual percentage change, AAPC)等指标分析1990-2023年中国儿童疥疮的疾病负担及变化。结果:2023年中国儿童疥疮的患病数、发病数和DALYs总数分别为8 274 136(95% UI 6 191 233~10 588 197)、25 427 952(95% UI 19 678 835~32 507 004)和217 005(95% UI 114 142~373 121)。1990-2023年,中国儿童疥疮患病数、发病数及DALYs总数在波动中整体呈下降趋势,而患病率、发病率及DALYs率在起伏中呈上升趋势。男性儿童疥疮患病率、发病率和DALYs率增幅略高于女性。10~14岁组儿童的患病率、发病率和DALYs率的点估计值在所有观察年份中持续居于最高,5~9岁组次之,而<5岁组最低。1990-2023年,中国儿童和全球儿童的疥疮疾病负担均呈上升趋势,但中国儿童的上升趋势相对更缓。结论:中国儿童疥疮的疾病负担仍较重,10~14岁儿童是未来防控的关键人群之一。

Abstract: Objective: To analyze the disease burden and trends of scabies among Chinese children from 1990 to 2023, and to provide evidence for its control and prevention. Methods: Data of scabies among Chinese children from 1990 to 2023 were collected from the Global Burden of Disease (GBD) 2023 database. Prevalence, incidence, disability-adjusted life years (DALYs), annual percentage change (APC) and average annual percentage change (AAPC) were used to access the changes of disease burden. Results: In 2023, the number of prevalent cases, incident cases, and total DALYs for scabies among Chinese children were 8 274 136 (95% UI 6 191 233-10 588 197), 25 427 952 (95% UI 19 678 835-32 507 004), and 217 005 (95% UI 114 142-373 121), respectively. From 1990 to 2023, the numbers of prevalent cases, incident cases, and total DALYs for scabies in Chinese children showed a downward trend with fluctuations. In contrast, the prevalence rate, incidence rate, and DALYs rate exhibited an upward trend with variations. The increase in the prevalence, incidence, and DALYs rates was slightly greater among male children than among females. Children aged 10-14 consistently had the highest point estimate for prevalence, incidence, and DALYs rates across all observed years, followed by the 5-9 age group, while the <5 age group had the lowest. From 1990 to 2023, the scabies disease burden showed an increasing trend among children both globally and in China; however, the upward trend in China was more gradual relative to the global trend. Conclusion: Scabies continues to pose a significant disease burden in Chinese children, with the 10-14 age group being a critical target for future prevention and control efforts.
