1990-2021年中国银屑病流行病学负担分析

摘要: 目的:分析1990-2021年中国银屑病流行病学负担趋势,量化年龄、时期、队列效应。方法:基于全球疾病负担(GBD)数据,提取中国银屑病患病率、发病率及伤残调整寿命年(DALYs)数据。Joinpoint回归计算年均百分比变化率;采用年龄-时期-队列(APC)模型分析疾病负担的时间趋势,并评估效应;分解分析量化老龄化、人口增长、流行病学转变贡献。结果:1990-2021年,中国银屑病年龄标准化发病率、患病率及DALYs率显著上升,年均变化百分比(AAPC)分别为0.74%、0.88%和0.88%,均高于全球同期增幅(0.27%、0.24%、0.29%),2021年分别达59.7/105、474.02/105和41.23/105。2021年男性负担高于女性,年龄分布呈双峰特征,分解分析表明DALYs增长主要归因于老龄化。结论:中国银屑病负担加速上升,具性别差异与年龄分层特征,老龄化是核心驱动,需强化防治。

Abstract: Objective: To analyze trends in the psoriasis disease burden in China from 1990 to 2021 and quantify the effects of age, period, and birth cohort. Methods: Using Global Burden of Disease (GBD) data, we extracted prevalence, incidence, and disability-adjusted life years (DALYs) for psoriasis in China. Joinpoint regression analysis calculated annual average percentage changes (AAPC). Age-period-cohort (APC) modeling assessed temporal trends and quantified effects. Decomposition analysis evaluated contributions from population aging, growth, and epidemiological transitions. Results: From 1990 to 2021, the age-standardized incidence rate, prevalence rate, and DALYs rate of psoriasis in China increased significantly, with an average annual percentage change (AAPC) of 0.74%, 0.88%, and 0.88% respectively. All these rates were higher than the global growth tates during the same period (0.27%, 0.24%, 0.29%). In 2021, the three rates reached 59.7/105, 474.02/105 and 41.23/105 respectively. In 2021, the disease burden of psoriasis in males was higher than that in females, and the age distribution showed a bimodal pattern. Decomposition analysis indicated that the growth of DALYs was mainly attributed to population aging. Conclusion: The burden of psoriasis in China is accelerating, with distinct gender differences and age stratification characteristics. Aging is the core driver, necessitating enhanced prevention and treatment.
