Clinical and pathological analysis of 8 cases with cutaneous angiosarcoma

Abstract: Objective: To investigate the clinical and pathological characteristics of cutaneous angiosarcoma (CAS). Methods: The data of CAS patients in our hospital from 2014 to 2024 were collected and analyzed. Results: A total of 8 patients with CAS were enrolled, all of whom were male. The age at diagnosis ranged from 67 to 91 years old, with an average of 78.3 years old. The time from onset to diagnosis was 1.5-7 months. Five cases had been misdiagnosed. The main clinical manifestations were erythema, plaques, nodules, bleeding after rupture, and scab formation. Histopathology mainly presented as varying sizes of vascular lumens within the dermis, partially overlapping with each other, with a single or multiple layers of endothelial cells that can form papillary structures into the lumen, and nuclear atypia. Those cells with low differentiation had unclear lumens, and tumor cells are spindle shaped and arranged in undles. Immunohistochemical showed CD31(+), CD34 part(+)/(-), ERG(+). During the 2-year follow-up, there were 3 deaths and 2 cases lost to follow-up. Conclusion: The clinical manifestations of CAS patients are lack of specificity and easy to be misdiagnosed. The disease progresses rapidly and the prognosis is poor.
