儿童线状IgA/IgG大疱性皮病一例

Linear IgA/IgG bullous dermatosis of childhood: a case report

摘要： 患儿，男，7岁。因躯干、四肢散发红斑、水疱伴瘙痒2个月就诊。发病前有泳池“暴晒”史。背部水疱组织病理：表皮下水疱，真皮乳头中性粒细胞及少许嗜酸粒细胞小脓肿，浅层中性粒细胞、嗜酸粒细胞、淋巴细胞浸润。正常人皮肤盐裂间接免疫荧光：循环抗体IgA、IgG于表皮侧均有沉积，局部区域IgG表皮真皮双侧沉积。综上诊断为儿童线状IgA/IgG大疱性皮病。给予甲泼尼龙、氨苯砜、人免疫球蛋白、吗替麦考酚酯治疗，病情逐渐好转。

Abstract: A 7-year-old boy presented with pruritic erythema and vesicular on his trunk and extremities for two months. He had a history of heavy sun exposure. Histopathology of vesicular revealed  subepidermal blister, microabscess composed of neutrophils and a few eosinophils in the dermal papillary and an inflammatory infiltrate of neutrophils, eosinophils and lymphocytes in the superficial dermis. Indirect IF using 1 M NaCl-split-normal human skin showed IgA and IgG autoantibodies deposited on the epidermal site. In addition, IgG autoantibody was also deposited on the dermal. The diagnosis of linear IgA/IgG bullous dermatosis of childhood was made. The patient was treated with methylprednisolone, dapsone, intravenous immunoglobulin and mycophenolate sodium and the condition was improved obviously.
