成人局限性朗格汉斯细胞组织细胞增生症一例

Adult localized Langerhans cell histiocytosis: a case report

摘要： 患者，女，32岁。外阴、肛周起红斑、斑块及溃疡伴疼痛1年余。皮肤科检查:外阴、肛周可见淡红色浸润性斑块,表面有溃疡、渗出，可见脓性分泌物。皮损组织病理示:真皮浅层见弥漫性淋巴细胞、嗜酸粒细胞及组织样细胞浸润。增生的组织样细胞轻度异型，部分胞核呈肾型。免疫组化检查：S-100（+++），CD1a(+++)，CD68 (20%+)，Ki-67(30% + )。诊断:成人朗格汉斯细胞组织细胞增生症。

Abstract: A 32-year-old female presented with erythemas，plaques and ulcers on the vulva and anus accompanied with itching for more than 1 year. Physical examination showed pale red infiltrated plaque with ulceration and exudates on the surface around the vulva and anus，and purulent secretions can be seen. Pathology showed diffuse infiltration of lymphocytes, eosinophilic granulocytes, and histiocytoid cells in the superficial dermis. The proliferative histiocytoid cells showed mild atypia，partly with kidney-shaped nuclei. Immunohistochemical examination showed that histiocytoid cells were positive for S-100 and CD1a，and weakly positive for CD68(20%+) and Ki-67(30% + ) . The diagnosis of adult localized Langerhans cell histiocytosis was made.
