慢性自发性荨麻疹患者外周血淋巴细胞ICOS/ICOSL表达及与免疫细胞的相关性研究

Expression of ICOS/ICOSL lymphocyte in peripheral blood of the patients with chronic spontaneous urticaria and its correlation with immune cells

摘要: 目的:明确慢性自发性荨麻疹(chronic spontaneous urticaria,CSU)患者外周血中Th1和Treg细胞的表达情况,观察诱导性共刺激分子(ICOS)及其配体(ICOSL)信号与外周血T/B细胞之间的关系。方法:采用流式细胞术分析外周血Th1、Treg细胞及其表面ICOS的表达情况,并对B细胞表面ICOSL进行鉴定。结果:患者ICOS+ Th1细胞和ICOS+ Treg细胞比例分别为(15.19±21.96)%和(60.04±31.82)%,显著高于对照组的(2.24±2.62)%和(48.72±19.13)%,差异均有统计学意义(均P<0.05)。Th1,ICOS+ Th1与嗜酸性粒细胞均呈正相关(r=0.4998,0.4701, 均P<0.05),ICOS+ Treg细胞与中性粒细胞呈负相关(r=-0.3771,P<0.05)。此外,Th1细胞与CD19+ B细胞以及ICOS+ Th1与CD19+ICOSL+ B细胞均呈正相关(r=0.5218,0.5616, 均P<0.05),而ICOS+ Treg细胞与CD19+ B细胞、CD19+ICOSL+ B细胞均呈负相关(r=-0.4183,-0.4519,均P<0.05)。结论:CSU患者外周血中ICOS+ Th1和ICOS+ Treg细胞比例显著增加,且与嗜酸性粒细胞、中性粒细胞和CD19+ICOSL+ B细胞之间存在相关性,提示了ICOS/ICOSL通路在CSU免疫调节中的潜在作用。

Abstract: Objective: To determine the expression of Th1 and Treg cells in patients with chronic spontaneous urticaria (CSU) and observe the role of ICOS/ICOSL signaling between peripheral blood T/B cells in CSU. Methods: The expression of Th1, Treg cells, and surface ICOS in peripheral blood, and surface ICOSL on B cells was analyzed by flow cytometry. Results: The proportions of ICOS+ Th1 cells and ICOS+ Treg cells in patients were (15.19±21.96)% and (60.04±31.82)% respectively, significantly higher than those in the control group, which were (2.24±2.62)% and (48.72±19.13)% respectively, and the differences were statistically significant (both P<0.05). Th1 and ICOS+ Th1 cells were positively correlated with eosinophils (r=0.4998, 0.4701, Ps<0.05), and ICOS+ Treg cells were negatively correlated with neutrophils (r=-0.3771, P<0.05). In addition, Th1 cells were positively correlated with CD19+ B cells, and ICOS+ Th1 cells were positively correlated with CD19+ICOSL+ B cells (r=0.5218, 0.5616, Ps<0.05), while ICOS+ Treg cells were negatively correlated with CD19+ B cells and CD19+ICOSL+ B cells (r=-0.4183, -0.4519, Ps<0.05). Conclusion: The proportions of ICOS+ Th1 and ICOS+ Treg cells in the peripheral blood of CSU patients are significantly increased, and their correlation with eosinophils and neutrophils as well as CD19+ICOSL+ B cells suggests a potential role of the ICOS/ICOSL pathway in the immune regulation of CSU.
