蜱咬伤致多脏器功能衰竭一例

摘要： 患者，男，58岁。5天前胸部皮肤被蜱咬伤后出现红斑、丘疹、肿胀伴痒，2天前前胸皮损出现边缘肿胀，伴胸闷气短，高热、寒战，恶心伴呕吐及阵发性晕厥急诊入院。D二聚体、血清肌酐、磷酸肌酸激酶明显异常，布尼亚病毒核酸检查阴性。诊断：(1)蜱咬伤；(2)多脏器功能障碍综合征。治疗给予糖皮质激素，创面处理等，20余天后患者好转出院。

Abstract: A 58-year-old male patient presented with erythema, papules, swelling with itching on his chest after tick bite 5 days ago. The lesions aggravated and the patient felt short of breath, high fever, chills, nausea, vomiting and paroxysmal syncope for 2 days. The indexes of d-dimer, serum creatinine, phosphocreatine kinase were obviously abnormal, and the nucleic acid test of bunya virus was negative. The diagnosis of tick bite and multiple organ dysfunction syndrome was made. The patient was improved and discharged after treated with glucocorticoid and wound treatment for more than 20 days.
