新冠疫苗接种后儿童中毒性表皮坏死松解症一例

摘要: 患儿,女,6岁。因“发热、咳嗽3天,皮疹3小时”入院,起病前接种新冠疫苗第2剂。患儿入院后皮疹进行性增多,且出现眼睛、口腔黏膜、肛周、外阴等黏膜破损,伴持续高热,头面部、颈部、躯干、四肢皮肤大面积表皮剥脱松解,受累面积占体表面积66%。诊断中毒性表皮坏死松解症。给予甲泼尼龙冲击、大剂量人免疫球蛋白、血浆置换等综合治疗36天治愈出院。

Abstract: A six-year-old female was admitted to the hospital due to fever, cough for 3 days and skin rash for 3 hours. She got the second dose of COVID-19 vaccine before the onset of the skin disease. After admission, her lesions increased gradually, and the erosion on her eyes, oral mucosa, perianal mucosa, vulva and other mucous membranes appeared, accompanied by persistent high fever. Epidermal necrolysis on her head, face, neck, trunk and limbs occurred. The affected area of epidermal necrolysis accounted for 66% of the body surface area. The diagnosis of a toxic epidermal necrolysis was made. She was cured and discharged from hospital after 36 days of comprehensive treatment including the pulse therapy with methylprednisolone, injection of high-dose human immunoglobulin, and plasma exchange.
