银屑病合并干燥综合征及抗合成酶综合征一例

Psoriasis with Sjögren's syndrome and anti-synthetase syndrome: a case report

摘要： 患者全身散在暗红色丘疹伴脱屑7个月余，加重2个月，合并口干、眼干，抗核抗体阳性，抗SSA抗体弱阳性，抗Ro-52蛋白抗体(+++)，抗PL-7抗体(+++)；涎液化糖链抗原KL-6 842 U/mL；胸部CT提示间质性肺炎；皮肤及唇腺活检提示银屑病和干燥综合征。根据临床表现及辅助检查诊断为银屑病、干燥综合征和抗合成酶综合征。经司库奇尤单抗和他克莫司治疗后恢复良好。类似病例目前尚无文献报道。

Abstract: This report presents a case of a patient hospitalized due to generalized scattered dark red papules with desquamation for over 7 months, aggravated for 2 months, complicated with dry mouth, dry eyes. Laboratory tests showed positive for antinuclear antibody, weakly positive for anti-SSA antibody, strongly positive for anti-Ro-52 protein antibody (+++)，strongly positive for anti-PL-7 antibody (+++); salivary glycoprotein antigen KL-6 was 842 U/mL; chest CT revealed interstitial pneumonia. Lesion and labial gland biopsy confirmed psoriasis and Sj?gren's syndrome.The final diagnosis was psoriasis, Sj?gren's syndrome, and anti-synthetase syndrome. The patient recovered well after treatment with secukinumab and tacrolimus. This case involves multiple autoimmune diseases including psoriasis, Sj?gren's syndrome, and anti-synthetase syndrome, which has not been previously reported in the literature. We summarize and report this case to enhance clinical physicians' awareness of such conditions.
