基于甲板分区定向掀甲切除甲下血管球瘤12例疗效分析

Efficacy analysis of a directional nail-flap elevation strategy based on nail plate partitioning and localization for excision of subungual glomus tumor in 12 cases

摘要: 目的:评价根据甲板分区定位选择掀甲方向的个体化术式在甲下血管球瘤切除中的临床疗效及甲板外观结局。方法:本研究回顾性分析了2022年5月至2024年7月在河北省沧州中西医结合医院手显微外科、足踝外科收治的12例甲下血管球瘤患者,术前根据病灶位置对甲板进行分区,并采用定向翻页式掀甲术切除肿瘤,使用视觉模拟评分(VAS)评估术后疼痛缓解情况、肿瘤复发情况,结合甲板形态评分及患者报告结局评估美学效果。结果:术后随访时间为8~24个月,12例患者末次随访VAS评分均较术前显著降低;术后甲板形态评分为(8.5±0.9)分,患者满意度评分为(9.2±0.7)分;末次随访时未见肿瘤复发及明显甲板畸形。结论:基于肿瘤位置的个体化掀甲切除甲下血管球瘤,可在充分暴露病灶的同时兼顾甲板美学效果。

Abstract: Objective: To evaluate the clinical efficacy and aesthetic outcomes of an individualized surgical strategy for the resection of subungual glomus tumors, which involves partitioning and localizing the nail plate based on the location of the subungual tumor and selecting the direction of nail-flap elevation based on the partition localization. Methods: This study retrospectively analyzed 12 patients with subungual glomus tumors admitted to the Department of Hand Microsurgery and Foot Surgery, Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine from May 2022 to July 2024. Preoperative nail plate partitioning was performed based on the location of the lesion, and tumor resection was completed using a directional flap-style nail elevation technique. The visual analogue scale ( VAS ) was used to evaluate postoperative pain relief and tumor recurrence, while aesthetic outcomes were evaluated using a Nail Morphology Score and Patient-Reported Outcomes ( PROs ). Results: The postoperative follow-up time was 8 to 24 months, with an average of 16 months. The VAS score decreased significantly from the preoperative value to that at the last follow-up. The postoperative Nail Morphology Score was 8.5±0.9, and the patient satisfaction score was 9.2±0.7. No tumor recurrence or significant nail plate deformity was observed during the follow-up period. Conclusion: Individualized nail elevation and resection based on tumor location for subungual glomus tumors can achieve adequate exposure of the lesion while preserving the aesthetic appearance of the nail plate.
