度普利尤单抗治疗获得性反应性穿通性胶原病一例并文献复习

Dupilumab in the treatment of acquired reactive perforating collagenosis: a case report and literature review

摘要: 获得性反应性穿通性胶原病(ARPC)是反应性穿通性胶原病(RPC)中最为常见的类型,其发病机制尚未明确,目前尚无统一治疗标准。本文报道1例常规外用及口服药物治疗欠佳的ARPC患者,予以度普利尤单抗治疗4周后瘙痒完全缓解,皮损变平,4个月后皮损基本消退,停药随访1年未复发。同时,本文对已报道的23例接受度普利尤单抗治疗的RPC/ARPC患者进行总结分析,结果显示,患者瘙痒症状多在治疗1~4周内显著改善,皮损平均清除时间为9.6周,且治疗期间未发生严重不良反应。本文病例及既往文献报道显示度普利尤单抗治疗ARPC安全有效,为常规治疗效果欠佳的患者提供了新的治疗选择。

Abstract: Acquired reactive perforating collagenosis (ARPC) is the most common subtype of reactive perforating collagenosis (RPC). Its pathogenesis remains unclear, and no standardized treatment protocol has been established to date. Herein, we report a case of ARPC with poor response to conventional topical and systemic medications. After 4 weeks of dupilumab treatment, the pruritus was completely relieved and the lesions flattened; the skin lesions almost resolved after 4 months of therapy. No recurrence was observed during 1 year of follow-up after drug withdrawal. In addition, we summarized and analyzed 23 previously reported cases of RPC/ARPC treated with dupilumab. The results showed that pruritus was significantly improved within 1-4 weeks of treatment in most patients, with a mean lesion clearance time of 9.6 weeks, and no severe adverse reactions occurred during treatment. Cases in this paper and previous literature reports have demonstrated that dupilumab is safe and effective for the treatment of ARPC, providing a novel therapeutic option for patients with poor responses to conventional therapy.
