Abstract: We report a case of gonococcal urethritis in a 45-year-old man who presented with a 3-day history of purulent urethral discharge, dysuria, and urethral pain. Urine was tested for N. gonorrhoeae RNA using simultaneous amplification and testing (SAT), with a positive result. Gram staining of the urethral discharge showed numerous polymorphonuclear leukocytes and intracellular Gram-negative diplococci. Culture yielded Neisseria gonorrhoeae, and antimicrobial susceptibility testing demonstrated susceptibility to ceftriaxone and cefixime. The patient was treated with a single 1 g intramuscular dose of ceftriaxone according to the Chinese guideline. At the 7-day follow-up visit after treatment, his symptoms had improved markedly, and urethral microscopy, culture, and repeat urine SAT were all negative.
