皮肤假性淋巴瘤一例

Cutaneous pseudolymphoma: a case report

摘要： 患者,男，13岁。右侧小腿红色丘疹5年，偶有瘙痒。组织病理示：表皮突萎缩变平，真皮浅层淋巴细胞、浆细胞团块状浸润，局部不典型淋巴滤泡形成，真皮中深部血管周围淋巴细胞、浆细胞浸润。免疫组化：真皮内浸润细胞CD3阳性，部分CD20阳性，浆细胞CD138阳性，kappa和Lambda散在阳性，呈多克隆表达，CD30阴性，Ki67阳性细胞约10%。符合假性淋巴瘤。治疗：外用丙酸氟替卡松乳膏。

Abstract: A 13-year-old boy presented with red papules on the right leg for 5 years with occasional itching. Histopathological examination showed epidermal process atrophy and flattening, infiltration of lymphocytes and plasma cell mass and local atypical lymphoid follicle formation in the superficial dermis, perivascular lymphocyte and plasma cell infiltration in the middle and deep dermis. Immunohistochemistry showed positive for CD3, CD20, plasma cell CD138, kappa and Lambda with polyclonal expression, negative for CD30 and the ratio of Ki67 positive cells was 10%. The diagnosis of cutaneous pseudolymphoma was made and the treatment of topical fluticasone propionate cream was given.
