以皮肤软组织和胸椎为主要损害的播散性隐球菌病一例并文献复习

Disseminated cryptococcosis with skin soft tissue and thoracic vertebra as main damage: a case report and literature review

摘要: 报道1例以皮肤软组织和胸椎为主要损害的播散性隐球菌病。患者,女,47岁。腰痛伴面部反复起丘疹3个月。皮肤科检查:面部散在直径大约5~7mm肤色扁平丘疹,部分中央有“脐凹”。皮肤组织病理:真皮急性炎性肉芽组织形成,可见较多中性粒细胞和组织细胞聚集,组织细胞内外可见散在圆形及卵形透亮小体,部分中央可见红色小球。免疫组化:AE1/AE3(上皮+),CD1a(-),CMV(-),EBER分子原位杂交(-),S100(-)、CD68(-)。特殊染色PAS、HE、六胺银染色均见大量孢子。皮损处渗液真菌镜检见大量宽阔荚膜的孢子。真菌培养见白色蜡样光滑菌落。分子生物学鉴定为新生隐球菌。诊断:皮肤隐球菌病;骨隐球菌病;溶血性贫血。治疗予胸12椎体病灶清除及口服氟康唑胶囊400 mg/d,随访1年面部丘疹及腰部肿物未复发。

Abstract: A case of disseminated cryptococcosis was reported. The patient was a 47-year-old female. Low back pain with recurrent facial papules for 3 months. Physical examination showed scattered in the diameter of about 5-7mm skin flat papules on the face. Skin histopathology showed acute inflammatory granulation tissue was formed in the dermis, with more neutrophils and tissue cells gathering, scattered round and oval bright bodies inside and outside the tissue cells, and some red globules in the center. Immunohistochemistry showed AE1/AE3 (epithelial +), CD1a(-), CMV(-), EBER in situ hybridization (-), S100(-), CD68(-). A large number of spores were found by special staining PAS, HE and hexamine silver. A large number of spores with broad capsular membranes were observed under the microscopy. White waxy smooth colony was found in fungal culture and.  identified as Cryptococcus neoforme by molecular biology. The diagnosis of skin cryptococcosis, bone cryptococcosis, hemolytic anemia was made. The patients were treated with 12 thoracic vertebra lesions cleared and oral fluconazole capsule 400 mg/d, there was no recurrence of facial papules and lumbar mass during one year follow-up.
