铁钉刺伤致皮肤普通变形杆菌感染一例

Cutaneous Proteus vulgaris infection after an iron nail puncture injury: a case report

摘要: 患者,男,69岁。左手背部红斑、肿胀、溃疡病史6个月。发病前除有铁钉刺伤史外,无昆虫或动物咬伤史。组织病理学检查结果为感染性肉芽肿。PAS染色未见菌丝和孢子。抗酸染色阴性。细菌培养阳性。经PCR扩增和16S rRNA测序确定为普通变形杆菌。综上诊断为普通变形杆菌皮肤感染。经盐酸米诺环素单药口服治疗3个月后皮损完全消退,随访2年无复发。

Abstract: A 69-year-old male presented with a 6-month history of erythema, swelling, and ulceration on the dorsum of his left hand. Prior to the onset, he had a history of iron nail puncture without insect or animal bite. Histopathological examination revealed infectious granuloma. No hyphae or spores were detected on PAS staining, and acid-fast staining was negative. Bacterial culture was positive. PCR amplification and 16S rRNA sequencing confirmed the pathogen as Proteus vulgaris. Based on the above findings, the diagnosis of Proteus vulgaris skin infection was made. The skin lesions completely resolved after 3 months of oral monotherapy with minocycline hydrochloride, and no recurrence was observed during a 2-year follow-up.
