利妥昔单抗治疗系统性红斑狼疮伴血液危象一例

摘要： 本文报道一例系统性红斑狼疮患者，血细胞三系呈进行性降低，伴有发热、乏力及鼻腔黏膜出血，甲泼尼龙从标准剂量治疗逐渐递增至500 mg/d（联合丙种球蛋白）冲击治疗，病情好转，但出现神经精神症状等不良反应，遂予糖皮质激素减量。2日后病情反复，伴鼻腔黏膜少量出血，加用利妥昔单抗治疗后病情好转。随访2年半，病情稳定。

Abstract: We report a case of systemic lupus erythematosus patient with progressive cytopenia, fever, fatigue, and nasal mucosal bleeding. Methylprednisolone was gradually increased from the standard dose to 500 mg/d (combined with gamma globulin). The patient's condition improved, but neuropsychiatric symptoms and other adverse reactions occurred. Two days later, the patient's condition recurred with a small amount of bleeding from the nasal mucosa. The patient was improved after the addition of rituximab, and his condition was stable after two and a half years follow up.
