Abstract: A 53-year-old male presented with recurrent skin erythema and subcutaneous nodules of lower extremities for 1 year, fever for more than 1 month. Hematology findings showed a peripheral blood eosinophilic granulocyte count of 4.92×109/L (55.3%), Epstein-Barr virus DNA showed positive. Skin biopsy showed a dense infiltration of mononuclear cells surrounded blood vessels, with atypic changes and a large number of eosinophil infiltration. Immunohistochemistry: CD3(+), Ki67(80%), TIA1(+), Granzyme B(+), Perforin(+), EBER(+). Biopsy of nasal mucosa and laryngeal masses showed heterotypic cells. The diagnosis of extranodal NK/T cell lymphoma (ENKTL) was made.
